Signal Passed at Danger (SPAD) Investigation
ITSRR is developing a suite of tools to help the rail industry better manage risk and improve overall safety management by identifying human factors that lead to occurrences.
Tools for investigating signals passed at danger (SPADs) are now available (see below). Similar tools are being developed for level crossings and worksite protection.
Signal Passed at Danger (SPAD) investigation
The Rail Safety Act 2008 requires rail transport operators to report SPADs to ITSRR using Form 6 Notification of Occurrence. ITSRR also encourages rail infrastructure managers and rolling stock operators to investigate SPADs using a systematic approach as presented in the flow chart below.

This approach involves using checklist-style forms developed for ITSRR by Human Engineering based on international better practice. They replace the old Form 5.
