International review of railway safety practices
The aim of the international safety practices review was to determine common railway practice in the five key safety priority areas of worksite protection, contractor safety management, level crossing safety management, compliance with infrastructure standards and management of signals passed at danger.
RISR 2011-12 Full report
This is the ninth NSW rail industry safety report prepared by the Independent Transport Safety Regulator.
Sharing practice: A discussion on the relationship between risk and asset management
The subject at the core of this paper is the relationship between risk and asset management. The paper provides background information on the subject, followed by some FAQs and answers.
Report on the Worksite Protection Systems Analysis Toolkit
The Worksite Protection Systems Analysis Toolkit supports the investigation of worksite protection occurrences. It facilitates the identification of factors contributing to human failure in worksite protection incidents when using the five forms of protection specified by rules in NSW.
Level crossing accidents in Australia
Collisions between road vehicles and trains at level crossings remain one of the biggest safety risks for rail operations in Australia, accounting for about 30% of rail related fatalities over the past five years. The human and economic cost of these accidents can be extreme, with individual collisions having the potential to cause multiple fatalities and damage costs exceeding tens of million of dollars.
Managing signals passed at danger
The approach to SPAD data collection, investigation and management differs greatly for rail transport operators (RTOs) across Australia and even within NSW. In recognising that a degree of consistency and improved data collection would be desirable, this information has been prepared to outline better practices used by some NSW RTOs and the UK overland rail network.
Guidelines for RRM
The Guidelines are designed to provide information to Accredited Rail Organisations (AROs) on the process for implementing and supporting RRM training.
